what should somebody do if they are put into secret services and given a new name and they slip up?

Probably contact the people that put them in witness protection...I'm sure if you are in such a program they brief you, etc.

To keep themselves safe they should contact the program and then the program will do what they think is correct. It also depends on what kind of a slip up your talking about.
