When people say "That hurt." Does that mean it really emotionally hurt them? Like they actually felt a stab of pain?

When I say that something hurt me. I actually feel a stab of pain inside my heart.

It could mean really hust like physical or emotional. But most ways I've heard it is in a sarcastic way.

It can be physical, emotional or sarcastic. It totaly depends on the context.
