How about a spa day?
Or tickets to see a show/concert/event that she likes?

my mom says to clean the house without being asked and make dinner for her (possibly whole family, not just you).

For my mom's birthday, I gave her a hotbath and made her a chocolate cake and played happy birthday on the piano ( I also put trick candles on the cake and she laughed). A spa day would be great too.

Hey I play happy birthday on the piano too :DD LoL :D
I think that every present is special. I used to buy my mother very expensive and big presents but not anymore.
For my mum, I usually never buy present like material present.
I give her what she wants. And thats a big hig and lots of kisses. And happy birthday on piano :D I think thats the best present for my mum.
Good Luck

Depends on what your mom likes too. My mom likes fairies & angels and books and stuff so I try to get her cool fairy jewelry (with her birth stone in it), or big sculpted cherub angel candles or books on topics she likes etc. If she likes a certain band/music/orchestra/play you can buy her concert tickets/CDs/passes for those.

wikawikawoo also got it right by doing something for her mum specially, like bake a cake and play her a song, and keep the house clean and take over all the chores in the house for a day...

Hey My mom said the best gift we (my bro and sis) gave her was we went to Kmart and got our pic taken. we don't really get together much we live in different parts of the state. the year after I got the pic laser printed into a charm. she hasn't taken it off since. The charm only cost 130.00 or so. but it was gold. and has had it over 3 years. So if you got siblings and it is possible why not. just split the cost of the pics...If not try to get her to talk about something that was special to her from her childhood. doll, toy, book, game. and try to find it or something like it there are a abunch of websites for vintage things. Bettysattic.com got some old stuff and that is the only one I can think of at the top of my head. Best of luck.
