What you have to think is about how the relationship is going. Is it going strong? Do you both trust each other? If you are having doubts about her thoughts or you feel this vibe she is confused and wants to get out of it, you should talk to her. The fact you found a break up note, must of left you terrified and upset. But now that she agreed to accepting your proposal and she explained how the note was from the past, maybe you should stress less and trust her. Because the more you are paranoid about it, it might leave her frustrated. If you however feel things arent going the way it used to, you must talk 2 her about it, gently and discuss things.
