well, first you have to get to know the girl that you have in mind of asking out. once you've realized that she's the one, maybe ask her out to dinner one night. do this as if you were just two friends going to dinner; people do this all the itme. you should just use a casual tone; don't sound flirty about it (this seems too forward for some girls).
but the key is confidence. if you have the confidence in yourself, and you are not negative, you'll most likely get a yes.
